Record the events of an incident with this report.

Accidents are never a good thing, however, when one happens, it’s important to document the events. Not only is this a matter of good practice, but in many jurisdictions it’s required by law. Safety inspectors may ask for incident or accident reports if follow up inspections are required. Use this accident / incident report to record the events for future reference.
