During an incident it’s important to maintain communication between your team and focus on ensuring that the situation is dealt with. Post incident however, it is equally as important to reflect on what occurred, and if possible determine what caused it and how it can be prevented in the future. Communicating this prevention measure to other people on your team and in your company is vital to prevention elsewhere.
Performing a lessons learned on safety incidents after they have occurred means reviewing the details of an incident and determing what they were and how they can be avoided in the future. All parties of the incident should be included in the review and analysis. By performing a lessons learned on incidents after they occur you will be demonstrating due diligence in the event they occur again.
In some districts performing these types of analysis are required by law.
